Protocol A: Initial Post-Op (Shoulder Arthroscopy)

  • Supine AAROM: Flexion with Opposite Arm

  • Supine Wand AAROM: Flexion

  • Supine Wand AAROM: External Rotation

  • Scapular Retraction

  • Standing Towel AAROM: Internal Rotation

 

Protocol B: Initial Shoulder Labral / Rotator Cuff Repair

 2 weeks after surgery:

  • Scapular Retraction

  • Supine Wand AAROM: External Rotation

4 weeks after surgery:

  • Supine Forward Elevation

  • Supine Wand AAROM: External Rotation

  • Self-Stretch: Shoulder External Rotation in Doorway (at neutral)

  • Scapular Retraction

8 weeks after surgery:

  • Start Physical Therapy

 

Protocol C: Total Shoulder Arthroplasty

Anatomical (TSA): Start 1 day after surgery:

  • Supine AAROM: Flexion with Opposite Arm

  • Supine Wand AAROM: Flexion

  • Supine Wand AAROM: External Rotation

  • Elbow Flexion AROM

  • Wrist AROM 4-Way (Gravity minimized)

  • Scapular Retraction

Reverse (rTSA): Start 2 weeks after surgery:

  • Supine AAROM: Flexion with Opposite Arm

  • Supine Wand AAROM: Flexion

  • Supine Wand AAROM: External Rotation (to 40deg)

  • Elbow Flexion AROM

  • Wrist AROM 4-Way (Gravity minimized)

  • Scapular Retraction

 

Protocol D: Golfer’s / Tennis Elbow

  • Self-Stretch: Wrist Extensor Stretch

  • Self-Stretch: Wrist Flexion Stretch

  • LAX Ball Soft Tissue Mobilization

  • Wrist AROM 4-Way (Gravity minimized)

  • Resisted Wrist Extension (Eccentric)

  • Resisted Wrist Flexion (Eccentric)

 

Protocol E: Capsular Stretches

  • Self-Stretch: Posterior Capsule / Cuff Stretch

  • Self-Stretch: Inferior Capsule Stretch

  • Self-Stretch: Anterior Capsule / External Rotation Stretch

  • Self-Stretch: Posterior Capsule (Horizontal ADD + Flexion)

  • Self-Stretch: Anterior Capsule / Extension Stretch with Dowel

  • Self-Stretch: Shoulder Scaption in Doorway

  • Side-lying Sleeper Stretch: Internal Rotation

  • Self-Stretch: Shoulder External Rotation in Doorway (at neutral)

 

Protocol F: Basic Shoulder Strengthening

  • External Rotation Side-lying w Dumbbell and Towel Roll

  • T-Band Resisted Shoulder Internal Rot

  • T-Band Row

  • Scapular Retraction w Elbow Flexion

  • Scaption + External Rotation with Dumbbell

  • Resisted Shoulder Flexion with T-Band

  • Resisted Shoulder Extension with T-Band

  • Resisted Shoulder External Rotation with T-Band

  • Resisted Shoulder Abduction with T-Band
