Arthroscopic Management of Osteoarthritis Shoulder


Arthroscopic management of advanced shoulder osteoarthritis (OA) is a minimally invasive joint preserving treatment using an arthroscope. Because it is minimally invasive there are postoperative swelling and only tiny incisions that allow for faster healing and earlier rehabilitation so you can return to your life and the activities that make life worth living.

This type of surgery is often recommended for young patients with severe shoulder OA to reduce pain and improve function. Young and active is defined as age 50 or younger.

It is a short-term fix for young active patients who are too young to undergo shoulder replacement surgery. This is due to the fact that joint replacements typically last 15 -20 years and return to play after a shoulder replacement typically requires three to six months.

OA is a disabling condition and significant cause of pain and dysfunction. While nonoperative treatment is often the first choice for treatment, when it fails, and the young and active patient has advanced OA of the shoulder, arthroscopic management of OA is a joint-preserving procedure.  It can improve function and reduce pain.

Why not have a total shoulder replacement?

These patients may have the option of a total shoulder arthroplasty, but the young active patient may prefer to delay that procedure to remain highly active.

The procedure

The procedure is performed under general anesthetia combined with local anesthesia.  The extent of the procedure depends upon the patient's needs. It may involve several procedures including but not limited to debridement of joint and articular surfaces, smoothing of the cartilage on the ball and socket, debridement, or removal of damaged tissue (fragments of broken cartilage and degenerated ligaments), and a capsular release to increases shoulder range of motion.

Recovery

The goal of rehabilitation is maintenance of joint motion, prevention of scar tissue formation and improved shoulder pain.

Arthroscopic management of shoulder OA is for younger, high demand patients who are not suitable for total shoulder replacement.  It offers reduced pain, improved function, and a predictable short-term joint-preserving option for these patients.
